The New York State Department of Health is responsible for the oversight and surveillance of Adult Care Facilities (ACF) in accordance with State laws, regulations and codes; as well as the investigation of ACF complaints. The Department conducts inspections for certification purposes every 12 to 18 months at each ACF, as well as follow-up inspections to ensure that any previously cited deficiencies are corrected. Additional investigations are conducted as a result of complaints and incident reports received by the Department.Pursuant to 18 NYCRR Part 486, the Department may assess Civil Penalties against Adult Care Facilities for violation of these regulations or of an order pursuant to subdivision 8 of section 460-d of the Social Services Law.
A total of 16 violations resulted from 4 inspections of this facility from January 1, 2022 through December 31, 2025, including 2 inspections resulting in no violations.
