**NOTE- TERMS IN BRACKETS HAVE BEEN EDITED TO PROTECT CONFIDENTIALITY** Based on observation, interview, and record review conducted during a Standard survey completed on 9/13/19, the facility did not eradicate and contain common household pests (e.g., small flies). Specifically, many small flies were observed throughout the facility. This involves Resident #75 and #114. The findings are: Review of of invoices provided by the facility from 6/2019 through 9/2019 from the Pest Control Services revealed there was no documentation of any services being rendered for black flies. On 9/13/19 the Pest Control Services documented service rendered for the facility treatment of [REDACTED]. 1. During an observation and interview on 9/6/19 at 10:16 AM, Resident #114 stated the flies bother me, and he swatted flies away from himself. Observation in main dining room during breakfast meal on 9/12/19 at 8:23 AM revealed empty meal carts were noted along wall outside of kitchen, when surveyor moved toward the cart, several small flies were noted to come off the cart. Further observation revealed many small black flies scattered on the white baseboard of the same wall. During an observation and interview on 9/13/19 at 10:00 AM, Resident #75 stated, the black flies are everywhere, I try not to have any fruit open, and I see them flying around my legs. I don't like bugs of any kind, I prefer they were not here! There were multiple flies were observed flying around the resident bandages legs Observation of Resident room [ROOM NUMBER] on 9/13/19 at 11:03 AM, revealed there were multiple small black flies in the room; including the two used urinals containing urine, the bed, and up the walls. During an interview on 9/13/19 at 11:32 AM, the Director of Maintenance, was not aware of any concerns regarding the black flies and would contact the pest control company to do an assessment of the facility. 415.29(j)(5)

Corrective Action For Residents Cited Herein:
Resident rooms for #75 and #114 have been deep cleaned for traces of food and other material/s that attract black flies. Additionally, all food carts have all been cleaned and instituted on a cleaning schedule as has the storage area they reside in. The wall was wiped down too as was the base board noted herein.

Identify Other Residents With Potential To Be Affected and Corrective Action AND Measures Put In Place And/Or Systemic Changes to Prevent Recurrence:
The Maintenance Director will round the entire facility and ancillary areas weekly and the Pest Control Company will round monthly. The Maintenance Director will inservice all staff by 11/08/2019 to report all concerns related to pest control immediately. The Food Service Director will inservice her staff to keep the food carts and storage area clean daily and after meals/meal delivery service. The Activity Director will review Food Storage Practices at the next Resident Council Meeting as well.

Monitor To Prevent Recurrence/QAPI:
The Maintenance Director will compile findings from all data sources noted herein and report monthly for a minimum of 90-days to the Committee on his findings. The Committee will determine the need for any additional process improvements or if the need for reporting needs to be extended.
